Our network includes thousands of independent eye care professionals, plus a nationally recognized retail optical network of familiar names including:

  1. The Cigna Fundamental Medicare (PPO) has a monthly premium of $0 and has an in-network Maximum Out-of-Pocket limit of $5,700 (MOOP). This means that if you get sick or need a high cost procedure the co-pays are capped once you pay $5,700 out of pocket. This can be a extremely nice safety net.   2. A copay (or copayment) is a flat fee that you pay on the spot each time you go to your doctor or fill a prescription. For example, if you hurt your back and go see your doctor, or you need a refill of your child's asthma medicine, the amount you pay for that visit or medicine is your copay.

Copay

When Cigna members visit a participating eye care professional, they pay only a minor fee (copay) for the vision examination and any amount over the stated allowances for materials. Cigna processes out-of-network claims within 10 business days of receipt of a clean claim and itemized receipt.

Members who choose to use an out-of-network professional pay that professional in full at the time of service and submit a claim form to Cigna Vision for reimbursement.
